The Advert Community Health Concierge and Senior Care Concierge were created with the goal of helping people age in place…remaining at home can be a reality!

Seldom does one NEED to go to a nursing home! Most care needed by older adults is for assistance with their normal daily tasks. An aide or companion can help with bathing, dressing, meals, laundry, medication reminders, light housekeeping and the like.

Our aides can offer one-on-one care to individuals in the comfort of their own homes. Our licensed social worker can speak to you about various options, including financial resources and entitlements that could be available.

Older adults experience difficulties with their “ADL’s”, activities of daily living. They need assistance with: bathing; dressing; bathroom needs; incontinence management; light housekeeping; laundry; companionship; shopping; escorts to appointments and the like. The Advert Community Health Concierge nurse aides and companions can help you with all of these things.

We are very careful in our screening process. We look for workers who have had experience in taking care of the elderly. They must do a face-to-face interview, submit references for phone and mail confirmation, (whenever possible), get a current TB test result, and a doctor’s release saying that they are healthy to do nurse aide work and have nothing on a statewide, Criminal Check. * As important as this screening process is, the fact that the workers get such a high portion of the charge for services strongly encourages a good work attitude and better care for you or your loved one!

In most cases, The Advert Community Health Concierge uses these terms interchangeably. We look for workers who have the right background and qualifications for your particular care needs’ situation. Many of our workers have had years or decades of experience.

There are some times when insurance dictates that we must have a CNA (certified nurse aide). We will work with you to do that.

The options are many and are on the increase! There are now more programs and creative ways of enabling people to remain in their own homes for their entire lives! There is a wealth of services, in-home care, equipment and financial resource alternatives that can help you to remain in your own home!   Owner of both companies and a licensed social worker can speak with you about your specific situation and acquaint you with options and ideas that may be appropriate for you to enable you to remain at home. She is also aware of programs and entitlements for which you may qualify.

We most certainly can! Our professional staff can discuss your situation with you. We can offer information, direction and/or specific services. If you need additional assistance in assessing the situation at large, looking at overall needs, coordination, management, services, legal and financial issues, entitlements and the like, we can connect you with one of our Geriatric Care Managers. The geriatric care manager can meet with you and/or your family in the privacy of your own home and help you determine what is needed, available and appropriate for your specific situation. The geriatric care manager can continue to work with you to assist with the coordination and management of your situation. This generally results in greater health and well being of the client and takes great pressure off of the care-giving family.

The Advert Community Health Concierge routinely offers care to younger adults with physical challenges resulting from disease, injury and/or conditions. We are here to help you remain as healthy and independent as possible.

The costs from The Advert Community Health Concierge are lower than most other home care companies, with the workers receiving a high portion of the fee. Our low prices cover ALL TIMES*, (evenings and weekends) and ALL TASKS! The Advert Community Health Concierge, Inc. has specific prices for hourly care, live-in care and brief visits. The cost for Care Registry workers is:

• *$21.50 per hour See Monthly membership plans for greater savings
• *$310.00 per live-in day
• *$45.00 per brief, one hour visit

We welcome calls to discuss your specific situation and care needs! *On major legal holidays, time and a half is paid to the aide.

Our fees are based on the amount of work that we do. As each situation is different, we charge by the hour and break that into 5 minute billing increments. Our geriatric care managers are all very experienced and have a wealth of knowledge to share with you. Our intervention can often save money, by helping people remain at home, connecting to services and/or receiving entitlements.

The Advert Community Health Concierge, Inc. has been approved countless times to provide care for those who carry long-term care (LTC) insurance. We will work with you to get the benefit approved, and even do the paperwork for reimbursement!

Yes! The Advert Community Health Concierge is one of the few providers, which offers live-in care. If the client is sleeping well at night, (getting up once or twice briefly), we can place a worker in the home that stays there and sleeps there one aide typically works 3 or 4 days and nights in a row and a second aide relieves the first to finish the week. We can do as little as one day of live-in care or up to seven days a week. Most home care companies will arrange for three eight-hour shifts and charge you between $400.00 and $500.00 a day!! The Advert Community Health Concierge’s live-in care costs less than $8.00/hour, ($290.00 a day!) and has only one shift change a week. Give us a call to discuss this option!

Yes! The Advert Community Health Concierge can provide help for as little as once a week, or as much as 24/7. We will work with you to develop a plan of care that works for you or your loved one. Give us a call to discuss your situation…. you are under no obligation.

The Advert Community Health Concierge and Senior Care Concierge are professionals who frequently work with hospitals to ease the discharge of loved ones to home. And licensed social worker or RN will work with you to explain what is needed, address issues about discharge, suggest services and equipment that can help, and speak with the staff at the hospital.

Most hospitalized patients can return home. We can speak with you about this goal and help you achieve it! Give us a call.

You can use The Advert Community Health Concierge or Senior Care for as little or as long as needed or desired. Our goal is to provide excellent care to you or your loved ones. We work closely with you to achieve that goal. With that in mind, you can just give us a call and let us know that the service(s) is not needed anymore.

We want to know whenever you have a problem or a concern! Our goal is your satisfaction!

We know that it can be difficult to adjust to needing care and/or to a caregiver. Whether there are small issues that can be easily addressed, bigger problems and/or personality mismatches, we will work with you to resolve the problem or change the worker. Please, don’t ever hesitate to notify us of problems or concerns. We are here to make this work for you and we are easy to work with!

Yes! We consistently provide that extra touch with one-on-one care to those who are in personal care or nursing homes, hospitals or rehabilitation facilities. This added care could be used to give comfort, supervision or address certain care needs. We can be there from one hour to 24 hours per day. Additionally, our geriatric care managers can oversee the transition from home to facility or facility to home. We can continue to be a resident advocate, attending care conferences, coordinating medical appointments/medical care, advocating for the hospitalized client and more. This can be tremendous help for the out-of-town family.

Yes! The Advert Community Health Concierge can place a worker(s) in your home when the care-giving family member wants to go on vacation, is hospitalized, needs to attend to important family business, and the like. This gives your loved one the ability to remain in familiar surroundings with one-on-one care. Give us a call to discuss your care needs.

Senior Care Concierge understands Guardianships. She has been appointed through the Court System to assume the role of Legal Guardian to an older adult. She has been responsible for personal and medical care decisions, financial decisions, end of life issues and more.

Our staff members are here to assist you if you are, or think you might become, the Legal Guardian of a loved one. Her staff can help guide you through the process, focusing on your loved one’s care management needs. We have assisted many family members who have the role of Legal Guardian or Power of Attorney and live at a distance from the loved one.

The Advert Community Health Project Concierge provides aides Medical Services, arrangements; and companions throughout USA Independent Contractors and Brokers register caregivers and medical provider welcome.
